Clutch Issue

Currently running at 160bhp, I replaced the clutch about 2/3 years ago when I got it remapped as was slipping, Recently the car felt juddery when pulling away and in reverse. Yesterday it was really bad made a loud slipping type noise and was really juddering. Today it was started to slip quite a bit and now its really a struggle to get it in gear. Is it time for a new clutch or could it be something simple? I noticed a whooshing noise when stationary when trying to engage the gears???
