Really rough zr oil leak?

So a few neighbours and friends have said when they hear my car coming it sounds rough. I took the car to a garage to get my belts changed and was told it sounds tappedey they check my oil and said it was sounding tappedley because it was on half on the dipstick so they refilled it but still sounds rough i check the water and oil every day and no change my temp in the car sticks at half way. The engine sounds like a tappeded i have had a look and noticed there is a bolt missing from my exhaust manifold the whole thread. From the inside it sounds like its sucking in air. So my oil doesnt leak or water doesnt leak my exhaust manifold is missing a threaded bolt. Could a exhuast manifold with gaps in make the car sound rough ? I am thinking its sucking in air the manifold making the car sound rough ??I have been told buyt family i can buy some oil cleaner which i can add to the engine and run for 15 mins which would unblock the oil valves going to the top of the engine? But i dont understand if there was no oil or low oil going to the tappieds my engine would over heat ?
